Registering with the Practice
When you attend the Practice to register you will be asked to complete a GP registration form, and new patient questionnaire form, these must be completed before you see the Doctor, so it is advisable to attend at least 10 minutes before your appointment time, so that you can compete the information required. Alternatively please print and complete these two forms:
Please present both forms at the Practice, where the receptionist will offer you a new patient appointment. Acceptance on to the list is as at the discretion of the Doctor. We will ask you for proof of identity(see below). (Medical Cards are not accepted as proof of identity).
Patients are advised to the read the NHS counter fraud declaration before signing the registration form.
There are a number of reasons why you may not be able to register with your chosen GP. For example, you may live too far away,if this is the case, simply choose another GP in your local area or you may not be entitled to NHS treatment (Please refer to Entitlement to NHS Treatment ).

Once you’ve been accepted as a patient, your medical records will be transferred to the Practice and you’ll be sent a new medical card. (Please refer to Medical card ).
Your medical records may take up to several weeks to arrive at the Practice.
If you default from your new patient appointment and do not inform the reception staff you may be refused acceptance on to the Practice list.
New Registration Policy
In accordance with Greater Glasgow & Clyde recommendations, all new patients should present two forms of identification which proves residency and authenticates their identity. This is required to reduce and prevent fraudulent attempts to obtain or misuse NHS care. Overseas visitors may be asked for additional information to prove NHS entitlement.
One form of identity must ideally be photographic such as:
- Passport
- Drivers Licence
- Official ID card from Public Services body
- Student matriculation card (current year)
Other documents for prove of residency that are acceptable are:
- Recent utility bill (within last 3 months)
- Council Tax document
- Television Licence
- Payslips (last two months)
- Rent book/agreement (Public Body or Private Landlord)
- Bank Statement (Name and address section only required)
- Solicitors Letter- (Clearly showing name and address)
If unable to provide photographic evidence then you must present one of the above and one of the following:-
- Birth Certificate
- Marriage Certificate
- Divorce\Annulment Papers.
THE ABOVE LIST IS NOT EXHAUSTIVE AND STAFF ARE ENTITLED TO ASK FOR FURTHER PROOF IF REQUIRED.
Dependants Oversea workers-Non EEA countries
Guidance given by Health Information Scotland ,the Health Care for Scotland factsheet(revised 2009) for people coming to study or work states:
“Staff at the GP surgery will ask you to fill in a form for your husband, wife or civil partner and any children. You may need to show your marriage certificate or civil partnership certificate and birth certificates for your children”

In order to complete the registration process we also ask that we are provided with a marriage/partnership certificate and/or birth certificate for your children along with any other relevant documentation, such as Passports, Visas etc. This is to satisfy oursleves that NHS entitlement has been met.
